In order to become a vendor at WIUX Culture Shock in Dunn Meadow on April 18, 2009, you must print and fill out this vendor contract and send it to:

These forms are due by February 29, 2009. A $25 entry fee is required to guarentee you a table at the event, cash or check (made payable to WIUX) only please. Any questions may be directed to Public Relations Director, Allie Korosi, at akorosi@wiux.org.
